1. Complete PTO Funds request form on this link.  25-26 PTO Reimbursement Form 

2. It will be submitted to Jay for approval.

3. Jay will let you know if he has any concerns.

4. Please keep in mind anything ordered at the end of the year must be received by the end of the 2nd week of June.

• PTO will reimburse you personally if the request has been approved by Jay.
