For LHHS alumni, an Immunization record was included in your diploma envelope picked up after graduation. In case of loss, please complete the Immunization Request option on the form above and send it to the registration office at malvizo@libertyhill.txed.net. This medical record must be picked up in person, showing the ID of parent/guardian or adult student.
