Physicals and medical history forms are valid for 13 months to allow a 1-month grace period as most insurances will not permit multiple physical exams within one calendar year.

How to sign Rank One Papers
To sign the electronic paperwork:
Go to https://libertyhillisd.rankone.com/Pro/Login.aspx to sign in OR create an account.
Once signed in, you will need the student's Last Name and Student ID Number.
From there on, you should be able to see their paperwork that needs to be completed.
To upload physicals - Rank One is very particular on how it accepts physicals. If the physical is 2 pages, please follow the instructions below:
If uploading a picture: please make sure BOTH sides of the physical are in ONE picture.
If uploading a PDF: Please make sure it has both pages attached to the file.
If you are having any issues, you can email the physical to us (lhhsathletictraining@libertyhill.txed.net) to upload it.

Before turning your physical in, please make sure the following is completed:
Each medical question is marked NO or YES
If marked YES, please make sure there is an explanation next to it.
Parent and Student signature at the bottom of the medical history form.
Physician marks in one of the clearance boxes.
Physician signs AND dates it on the physical exam form
