Physicals and medical history forms are valid for 13 months to allow a 1-month grace period as most insurances will not permit multiple physical exams within one calendar year.

At the end of the school year, Liberty Hill ISD will be hosting physicians and other healthcare professionals to perform and complete the pre-participation physical exam that is required annually by the school district.

No physical exam will be performed if the Medical History side of the form is incomplete (including signatures).
