Below are the standardized forms required to avail various financial services from our office. Kindly download the relevant form, complete all required fields accurately, and submit it as per the instructions provided. Please ensure the information is filled in correctly to avoid rejection or delays in service delivery.
1. Salary Advance Form - Submit a completed hard copy of the Salary Advance Form. The request will be verified for outstanding advances, approved by the Head of CFS, and processed within one working day, subject to budget availability. Eligible once per financial year, up to one month’s gross salary.
2. Pay Slip Issuance Authorization Form - Submitting a pay slip issuance authorization to a third party is necessary to give official permission and protect employee salary information. It ensures that the third party can securely access and provide pay slips while maintaining confidentiality.
3. Leave Request Form -Used to formally request leave.
4. Note for the Sanction of Expenditure - This form must be submitted and approved before executing any planned activities involving expenditure. It serves to obtain authorization, confirm budget availability, and ensure compliance with financial regulations.
5. Tour Report Format- Claims must be accompanied by a completed tour report using the prescribed format. Ensure all required details are accurately filled out before submission to avoid delays in processing.
6. Travel Authorization Format - Submit the completed Tour Authorization Form in advance to seek approval and confirm budget availability. Forward the approved form to the Travel Admin to initiate the authorization process in e-DATS.
