Checklist for FIVE Requirements

My child(ren) will:

• embrace the community of an online learning environment.

• have access to a computer (personal or District 5 issued Chromebook).

• have access to high speed internet.

• participate in live virtual instruction daily.

• have an adult available for support in the virtual learning process to monitor student engagement of instruction and completion of assignments and assessments.

• have a dedicated space for school work each day.

• participate in class and interact with the teacher and peers daily.

• remain in FIVE for at least one quarter (elementary) or semester (secondary).

• attend FIVE daily and follow attendance guidelines when absent. I understand daily attendance will be monitored.

• report in person for mandated testing. Personal transportation will be required for onsite assessments.
