2) You must get written parent permission.

*If mentor is doing some sort of sports mentoring, then your mentor must also get a TB test prior to starting. Contact bludlam@lexington1.net for the necessary paperwork.

1. What do I need to do if my mentoring program includes an activity like fishing?

- Make sure to include a question in your parent permission slip about whether or not the child participating in the mentoring program can swim.
- Ensure that there is an adult who is certified in CPR and First Aid and there is access to a working phone in case of emergency.
- Make sure there is a First Aid kit on site.
- Make sure that mentors can easily access school personnel that have each child's emergency contact information.

2. What do I need to do if my mentoring program includes sports?

If you are using mentors that are not current employees from Lexington One, ensure that in addition to background checks, they also have TB tests completed.
Ensure that there is an adult who is certified in CPR and First Aid and there is access to a working phone in case of emergency.

3. What if a child makes a suicidal comment to a mentor?

The mentor needs to immediately ensure that the child is supervised by a school staff member and report that comment to a school counselor.

4. What if the mentor suspects that a child is being abused?

The mentor needs to speak (before leaving campus that day) to a school counselor or building administrator.
