Where: Lebanon High School Gymnasium
Cost: FREE - If you do not show up for your appointment you will be charged a no-show fee $25 per student.
Grades: Current 6th-11th graders (grades 7-12 for the 26-27 School Year)
Required Forms: PIAA section 4 must be completed prior to your appointment on May 18th. Section 6 will be completed at the physical by the medical provider.
