WEX***For your CAPSTONE presentation...please remember to provide a copy of your work experience for your CAPSTONE presentation (this could be a photocopy, scan or photo of the 1st page of your completed orange form). You should also attach a copy of your answers to the 7 "Evidence Of Learning" questions below (the original copy should be attached to your orange form which must be handed in at the Career Counseling Center). Please feel free to add a paragraph or two describing your work/volunteer experience.

You must complete the Orange Work Experience 12 forms, including the Training Plan, the reflective "Evidence of Learning" questions, the Evaluations and the appropriate signatures. If you are completing this form for Capstone 12 and Work Experience 12, you will need 100 hours of work. If you are completing this form only as a Capstone 12 requirement, you will only need 30 hours of volunteer work. Every Grade 12 students must complete this form. You may include the hours that you have accumulated from the summer prior to grade 10 until the completion of grade 12.
THIS IS A MANDATORY GRADUATION REQUIREMENT!!!

We will need the following pages filled out:
Career Conversation – Please tell me how this job will help you in your future or future employment.
Training Plan – you fill this out completely (take a picture of the complete page for your Capstone).
Safety Checklist completed by you and your employer together.
Evidence of Learning – you will type your responses to these questions on a word document, print it off and staple to the orange sheets (save a copy of these answers for your Capstone).
Evaluations – fill out your student evaluation 1st, then ask your employer to fill out the employer evaluation.
Hand in the completed documents with your printed responses to the "Evidence of Learning" questions stapled together into the Career Counselling Center.
