Supply List

PRE-ATHLETICS & PE UNIFORM STORE - Opened until March 25th, 2024. 

(Store/uniform is optional and run by the Four Points Athletic Boosters.)

*Students must change clothes in Lifetime Fitness & Pre-Athletics classes. They cannot wear the same clothes they wore to school during Lifetime Fitness & Pre-Athletics classes. We require students to change into black shorts with an elastic band and of appropriate length (no blue jean or button dress shorts). Cutoff and sagging shorts are not acceptable. Students must wear a gray t-shirt that is appropriate for school and free of offensive and/or suggestive material written or pictured on the shirt. Students may also wear the uniform purchased by the Four Points Athletic Boosters. Athletic shoes must have rubber-soled bottoms and laces. Flip-flops, sandals, boat shoes, Vans, Converse, hikers, and boots of any kind are not acceptable. Students with hair long enough to be pulled back must have a hair tie and pull hair back in class. We strongly encourage students to bring non-spray deodorant, soap, towel, and any other grooming supplies necessary for proper hygiene. Aerosol containers are prohibited at Four Points Middle School.

*Clothes worn during Lifetime Fitness & Pre-Athletics must be taken home to wash on the last day of each school week

Health (6th Grade)

Students will be taking approximately six weeks of health education on the following concepts:

Grading Policy

The grade in Lifetime Fitness & Pre-Athletics is based on class effort/participation (75%) and uniform wear (25% in-person learning only). Effort/participation includes assignments, punctuality, following directions, sportsmanship, attitude, behavior, and health (6th grade). Both effort/participation and uniform wear will be assessed on a daily basis, which will result in a weekly grade for each category. 

*Parent communication is required for non-participation in Lifetime Fitness class. Please provide a note or email to communicate any health issues that will lead to non-participation or modified participation. If a student will not be participating for more than three days, please provide a doctor’s note. 

Locker Room 

7th & 8th Grade Lifetime Fitness students will not be provided a locker (unless requested by parent). 7th & 8th Grade students must bring their clothes for Lifetime Fitness class each day in their back pack. 7th & 8th Grade students will be allowed to take only their uniform clothes into the locker room to change, and then they will bring their school clothes back into the gym and put them into their back packs until the end of class. In the gym, students will leave back packs and other large items in the front of the gym next to the coach's desk. 

6th Grade students will be provided a locker. 6th Grade students must supply their own combination lock, not a key lock. 6th Grade students will give their respective coach the combination to their lock. It is the student’s responsibility to lock up his/her belongings during class time. 6th Grade students can only bring items into the locker room that can be locked in their locker. The lockers in the locker room measure 12H X 12W X 12D. Each student must replace his/her own lock if it is lost or breaks. 

The locker room is strictly a technology free area. Cell phones should not be seen out inside the locker room for any reason. The gym is also a technology free area. Computers, cell phones, and other electronic devices are not allowed in the gym without instruction by a coach. Technology devices seen by a coach in the locker room or gym will be confiscated and turned into the front office.

Items left out in the locker room will be picked up and taken to the school's lost and found location in the FPMS Cafeteria daily. If a student has an item that has been lost or stolen, he/she should immediately report the item(s) to the coach. Action will be taken to recover lost or stolen items. 

Students who do not show responsibility in the locker room, such as leaving clothes lying around, will conference with the coach.

FPMS is a CATCH (Coordinated Approach to Child Health) school.
