Read this for an overview: “Aggressive Monitoring: Analyzing and Responding to Real-Time Classroom Data.”
Then watch these two examples of teachers using the strategy in class while students work independently:
ELA middle school teacher (3.5 minutes long)

Create feedback codes--make a poster or draw on board so students see codes. Sample
Create pathway--may involve adjusting seating chart (could use tool in PowerTeacherPro).
Create lap sheet to track learning. Samples/Templates:

Check the standard and review assessment framework.
Complete the independent work task as if you're the student to identify what exemplar student work includes and to identify where students might develop misconceptions.
Turn what you notice into at least three natural pauses in the task that address both work quality and work strategies. Those will become what you inspect during each lap of the room.
On your lap sheet, identify those three pauses as the title of the lap. Have the sheet on a clipboard you can carry.
Transition to independent work, explaining the feedback codes to students (we can skip this step as students get familiar).
Carry clipboard with lap sheet and pen, announcing each lap task to class.
Follow pathway plan around room to monitor students' work and add feedback codes to student work.

At end of lap,
if many students are struggling, it's a time to reteach whole class.
