Below are the Fillable Forms for students and parents to complete and submit. To access and utilize fillable fields:

1. Open document, click link to download (upper right corner of screen)

2. Click on ^ icon at bottom left of screen to open download, and select "open with system viewer"

3. Complete all fields, then "Save As" in the File drop down menu at top left of screen.

4. Once complete, please email both Senior Worksheet and Transcript Release as attachments to Mrs. Strother lisa.strother@lcps.org

5. When sending in your first submission, include "Senior Worksheet" in subject line of email.
