First the student ID number must match the last name of the athlete on the forms being submitted. The forms can't be completed if these do not match. 

To access the online forms click either "continue as guest" or create/login to your profile. From here you will use your student ID number and find your athlete.

There are a few forms you must submit electronically; you must print the Pre-participation & Medical History Forms and take them to the Dr.’s office on the date of the physical examination. Once the physical is signed and dated by the doctor, please scan the SECOND or BACK page and upload it to RankOne.

Click on the form name and fill out the information requested. To sign the document click inside the signature box and hold your mouse down; this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. Enter a valid email address at the bottom of the form. You will receive a confirmation email once each document has been submitted and received by the RankOne system.
