When a child is enrolled into LBUSD a health file is created for the student.

The following documents needed includes:

1. The Permanent Student Health History form*  (Completed in ParentVue)

2. A copy of the most updated immunization records*

3. Report of Health Examination for School Entry Form Stamped by Doctor's Office ** (CHDP)

   1. • Physical examination within 18 months prior to or 90 days after entering first grade.

4. Dental/Oral Health Assessment Report Form Stamped by Dentist Office** (OHA)

   1. • Oral health assessment within 12 months prior to or 8 months after entering kindergarten or first grade (whichever is their first year of public school).

Instructions (Please read carefully): 

Download the forms and print.

• Parent/guardian of the enrolling student will need to fill out the Permanent Student Health History Form.

• Student's doctor will need to fill out the Report of Health Examination for School Entry.

• Student's dentist will need to fill out the Oral Health Assessment Report

• Prepare copies of the immunization records. Verify with the student's doctor that all immunizations are completed, or you may refer to the Immunization Requirements tab on the top of this webpage. For further help, contact either your school's enrollment clerk or the school nurse.

Once all forms are fully completed, scan and email the forms to the school's enrollment clerk or the school nurse. You may also drop off the hard copies of the completed forms to the front office of the school.
