Immunizations Required Upon School Enrollment in LBUSD:

Ages 4 year old and older:

5 Doses of Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td)

4 Doses of Polio (OPV or IPV)

3 Doses of Hepatitis B (HepB)

2 Doses of Measles, Mumps, and Ruebella (MMR)

2 Doses of Varicella (Chickenpox)

For 7th Grade Entry

1 Dose of Tdap Booster

1 Dose of Varicella (only if records show 1 immunization)

Personal Belief Exemptions of Immunizations are no longer accepted upon enrollment in LBUSD.

If your child has a medical reason that requires exemptions of immunizations,

please contact your school nurse for further instructions.

ALERT! As of January 1st, 2021, there have been updated guidelines for exemptions of immunizations due to a medical reason. The student's physician is required to generate an immunization exemption letter to include a CAIR-ME code. For further details and instructions please refer to the link below.
