In order to attend school in California, students must show written proof of all school-required immunizations. Current requirements can be found here in ENGLISH, SPANISH and other languages.
All 7th graders must show proof of the following BEFORE the first day of 7th grade:
Tdap booster immunization
Two doses of the Varicella immunization
If you have received a message from the school regarding this issue, please follow the instructions in the notification and provide the missing paperwork as soon as possible. If you have questions or concerns, text Nurse Gibson at 562-999-2121 or email her at cagibson@lbschools.net
Does your student need an inhaler or other medication during the school day? Please note, students are NOT allowed to carry medication unless certain forms are completed EACH school year.
This paperwork makes sure our school nurse is aware of your student's health issues and can quickly help if needed. Click the "Forms" tab above or scroll down for medication related forms below.
Please contact the school nurse if your student has...
Had a major illness, major accident or hospitalization
Received a NEW medical diagnosis
Started NEW medication or changed their current medication
If your student has had a significant change in their health, please complete and turn in a new Permanent Health History (ENGLISH/SPANISH)
DID YOU KNOW... physical activity & movement help you do better in school?!? Students (and adults too) who use frequent movement breaks tend to be better at learning new things! It's true!
Nurse Gibson says... do your body & your mind a favor, every 30-60 minutes... MOVE, STRETCH, look around, take some deep breaths, and move some more! As they say, "Use it or Lose it!" And when it comes to snacking, try to eat things that might GROW on a farm and less of those things made in a factory.
