MEDICATION FORMS

Does your student need an inhaler or other medication during the school day? Please note, students are NOT allowed to carry medication unless certain forms are completed EACH school year. This paperwork makes sure our school nurse is aware of your student's health issues and can quickly help if needed. 

If you want your student to carry their medication, please complete this form:

PLEASE NOTE - Medication paperwork will not be accepted by the front office unless it is...

1. Turned in by an adult

2. Completely filled out with a signature from a guardian AND signed & stamped by healthcare provider 

3. Turned in WITH the ordered medication
