Registration & Form Orders

The DMV registers organizations to participate in the ID card programs. To apply:

1. Complete the DL 932 Order Request Reduced Fee or No Fee identification Card Program (PDF).

2. Nonprofit organizations must submit the required documentation.

3. Email all requests to: dmv_lod_ppd_reducedf@dmv.ca.gov.

4. Once approved to participate in the ID card programs, organizations will only need to submit their DL 932 Section 1-3.
