Physicals

All student athletes must complete a physical. Their doctors can fill out their own form that states that students are cleared to play sports. Otherwise, they can use the forms below.

Families can also be seen by Dr. Konstantin, who has worked with the league for many years and accepts walk ins. Bring the form and $20 to:
