Assumption of Risk Policy

I have chosen to have my child participate in dance and/or Gym instruction given by Lakeview Creative Arts. I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Lakeview Creative Arts is not in any way responsible for making such a determination.

Auto Withdrawl Policy

If your payment is not received by the due date and your form of automatic draft is declined, your account must be brought current within 7 days, or your student will be dropped from all classes, and enrollment will be canceled. At this time, an invoice will be mailed to the address on file, and all efforts to collect the past-due balance will be explored.

Behavior Policy

All families are expected to follow all studio policies and be courteous and respectful to all members, faculty, staff, and each other at all times.  Disruptive behavior from any student will result in a parent-teacher-studio director conference.  Should the behavior continue after the parent-teacher-studio director conference, the studio reserves the right to remove the student from our rolls.  No refunds will be issued if the student is dismissed due to behavior.

Class Viewing Policy

Parents will have the opportunity to watch class via the in-lobby monitors for one week in October, December, and February.  

Communication Policy

I understand that the PRIMARY source of communication between myself and the studio is email, and the email address I have provided is one I check regularly. My first contact should be the studio's office staff, which may be reached via email at info@lakeviewcreativearts.com. Facebook and Instagram messages are not checked as often and should not be used for current students to contact the studio with questions. Studio staff may take up to 24 hours to return emails; as such, I will not call, email, and/or message social media simultaneously for an instant response. Doing this may result in incorrect information and/or miscommunication.

Dress Code Policy

All families are expected to follow the Lakeview Creative Arts class Dress Code which may be found here.  If a student comes to class without being in the dress code, a reminder/warning will be sent home.  Should this occur a second time, the student will not be able to participate in class but will be allowed to observe class.  Any subsequent occurrences will result in the student not being allowed into the studio for class.

Lobby Policy

Our lobby allows parents to wait for their dancers.  Those staying in the lobby should remain quiet so as not to disturb ongoing classes.  Should the noise level of the lobby become distracting or too noisy, LCA Staff reserves the right to close the lobby and/or to ask those causing the distraction and/or noise to step outside.  What is considered "distracting" or "too noisy" is at the discretion of the LCA Staff.  No parents will be allowed in the studio during classes.

Make-Up Policy

We understand that schedules are ever-changing and it may become necessary for you to miss class.  Please call or email the office at least 24 hours before the start of class to notify us.  If an appropriate makeup class is available, a makeup will be scheduled.  No refunds will be offered if a student misses class.

Modification Policy

Should any of these policies be changed during your enrollment at Lakeview Creative Arts, you will be notified via email and upon signing in to the parent portal.  If you wish to modify any part of what is seen here, including any of our policies as a whole or portions of individual policies, you must discuss it with an authorized representative of Lakeview Creative Arts prior to registration.  Modification of our policies through the notes section of the registration form or student portal account is not accepted as a modification, change, adjustment, or counter-offer.  Should Lakeview Creative Arts agree to make a policy exception, a separate policy will be drafted and signed before registration can be accepted.

Photo Release

I grant Lakeview Creative Arts the right to take photographs and videos of me and my family in connection with lessons at Lakeview Creative Arts. I authorize Lakeview Creative Arts, its assigns, and transferees to copyright, use and publish the same in print and/or electronically. I agree that Lakeview Creative Arts may use such photographs, videos, or likenesses of me and/or my family as needed for any lawful purposes, including but not limited to such purposes as publicity, illustration, advertising, and Web content, and that I have no rights to the same and will not be compensated for the same.

Privacy Policy

We will not share your information with any third-party vendors.  Taking pictures or video of the monitors in the waiting area or turning on said monitors without the expressed authorization of a Lakeview Creative Arts staff member is not allowed at any time.  This is for the privacy and protection of all families and students within our school.  We appreciate your understanding.

Recital Policy

It is assumed that all students will participate in the recital held at the end of the dance season, and all fees and costumes associated with the end-of-season performance will be due regardless of your ability to participate.  Exceptions are given in very limited circumstances at the discretion of the LCA staff.  All requests for exceptions must be received by December 15th and should be made via email to elizabeth.stiegman@lakeviewcreativearts.com.  Texts, calls, and verbal requests to staff will not be an accepted method to decline recital participation.

Release of Liability

In consideration of my child's enrollment in any dance and/or gym instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Lakeview Creative Arts from all claims, costs, liabilities, expenses or judgments, including attorneys' fees and court costs for any occurrences in connection with any dance and/or gym instruction. I assume all risks to my child in connection with any instruction and further release Lakeview Creative Arts and its owners and employees from liability for any injury sustained by my child while he or she is enrolled in any dance and/or gym instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen. I understand that Lakeview Creative Arts is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio and that Lakeview Creative Arts will only be supervising my child when he or she is participating in scheduled dance and/or gym activities, programs or instruction. I understand that Lakeview Creative Arts is not responsible for personal property that is lost, damaged or stolen while I or my child is at or on Lakeview Creative Arts property. I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child participating in Lakeview Creative Arts activities and that Lakeview Creative Arts does not provide accident or health insurance for those participating in its instruction, activities or programs. My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation. I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND HAVE READ THE SAME PRIOR TO SIGNING.


Tuition and Registration Fee Policy

All students have a $50 non-refundable registration fee due at the time of registration.  There are two payment options for tuition.  Families may choose from the Semester or Full Year payment options.  For the Semester Payment option, 50% of tuition is due at registration; the remaining balance is due on January 1st.  Those choosing the Full Year payment option will receive a 5% discount on tuition fees, and payment in full is due at the time of registration.

Uncontrollable Closure Policy

Lakeview Creative Arts will make every effort to be open while considering the safety of our students and staff. There are situations that can occur beyond our control that make remaining open impossible or unsafe. When possible, students will be scheduled for a makeup class within 6 months of the closure date(s). In the event of adverse weather conditions, such as tropical storms, hurricanes, freezes, snow, etc.... Lakeview Creative Arts will follow the Orleans Parish school closures as a guide for closing and canceling classes. If evacuations are suggested/ordered and remain in effect for a prolonged period of time, or if people are unable to return home due to weather conditions, the studio will remain closed until it is deemed safe to return by local authorities. If we are unable to reopen due to damage to our building, and/or the landlord determines we are unable to reopen for any reason, no refunds will be given for tuition already paid to the studio. In the event of unforeseen closures, a message will be listed in your Parent Portal. We will also post closures to our Facebook and Instagram pages. It is every parents' responsibility to check emails and the Parent Portal on a regular basis to remain up to date with all studio events.

Withdrawal Policy

Withdrawal Policy: It is assumed that dancers will remain a student for the duration of the dance season.   If you need to withdraw, the notification must be emailed to elizabeth.stiegman@lakeviewcreativearts.com.  If withdrawal after the season begins becomes necessary, and all efforts to adjust class times have been discussed, there are no refunds.  However, credit for unused tuition may be applied to your account for future classes.
