Scheduling Forms

Please note that completed forms are required for enrollment in the below classes.  All forms for classes happening the following school year must be submitted by the last day of school for the current school year. 

Advanced Placement (AP) Contract - Required for any student taking one or more AP courses the following year.

Background Check - Required for any student taking wishing to take any of the following classes: Certified Nursing Assistant (CNA), Medical Assisting, and Human Services.

Diploma Programme (DP) Contract - Required for any student wishing to take a DP course or complete the DP/CP program.
