PHYSICALS

The Texas UIL requires each student to have a physical form on file before he/she can participate in a sport/physical organization. Copies of this form can be picked up in the Athletic Training Room, room 1623. These can be filled out at a family doctor or at our annual campus physical screening, but must be updated annually. These forms provide the coaches and trainers information on each athlete's physical condition and emergency contact information should an emergency arise.

RANK ONE FORMS

These forms must also be completed and turned in before a student can participate in any sport/physical organization. These can be done on paper, or electronically on the Klein ISD website using the link below.

Official Statement for Physicals 2022-2023:

Klein Cain Hurricane Athletics is very excited to announce we will once again be hosting, in conjunction with our partners at Houston Methodist Orthopedics and Sports Medicine, a date for student-athletes to receive a UIL physical at Klein Cain High School, on April 28th, 2021, from 2PM-6PM, for the cost of $20 per student, cash or check, paid at the door.

Due to the need for public health accommodations and social distancing, we will only be doing physicals via pre-booked appointments. Unless your child received a physical after August 2020, they will need a new one to begin participation in athletics for the 2021-2022 school year.

to schedule an appointment for their child to receive a physical.

-After going to the link, you will need to select “KLEIN CAIN/ULRICH/DOERRE PHYSICALS @KLEIN CAIN ON 4/28 FROM 2PM-6PM - $20 PER STUDENT” from the classes option, and select a time for your student (there are currently 18 openings every 5 minutes, from 2PM-6PM).

-After selecting your time, you will need to submit your students name, ID number, parent email address, and the name of the school your child is currently attending, and you will receive a confirmation email for your appointment time.

On the day of physicals, students will need to:

-Wear a mask and follow all KISD public health guidelines

-Arrive no earlier than 10-15 minutes before their appointment time (entrance will be Klein Cain Auxiliary Gym)

-Bring filled-out Methodist Consent Form (provided on back page), we cannot provide physical without filled-out consent form and you will forfeit your appointment

-$20 cash or check
