We kindly remind parents and guardians of the importance of completing the mandatory forms—Use of Images (Consent Form) and Medical Questionnaire—prior to your child’s enrolment. These forms are provided below for your convenience. Submitting these forms in advance is essential, particularly the Medical Questionnaire, as it allows our school nurses to be well-prepared to support your child’s health and safety needs from their very first day with us. 

For our boarding students, parents are encouraged to complete the Intention to Board Form and either email it directly to the respective Houseparent or submit it to us in person on Induction Day.

To ensure a smooth and efficient process, we kindly request that the following forms be submitted prior to Induction Day:
