STEPS TO TAKE TO INITIATE AT SERVICES
1. Complete the Initial AT Support Request Form: This form will be sent to the LATS (Local Assistive Technology Specialists: Kristy Linares and Kyle Sheer) who will review it and contact you with further steps.
2. Review the AT Consideration Checklist and the AT Resource Guide By Subject in the area of need for possible AT solutions to try first.
3. Work with your LATS to receive equipment, training, and information on how to gather data on the use of necessary AT trials.
4. Complete the Assistive Technology Trial Data Form for the area of need.
