Ready to Go? If You're a Student...

Not so fast... We need you to do a few things.

There are some forms that must absolutely be completed by ALL travelers to whom they apply. Look at the list below to determine which forms apply to you. You may print them yourself (links to each provided) and bring them to us once they're completed. We will also go over this checklist with EACH traveling student to ensure that they know what to bring to parents, guardians, and/or medical professionals for completion.

ALL STUDENTS PARTICIPATING IN ANY FIELD TRIP MUST COMPLETE THE FOLLOWING FORMS

RESPONSIBILITY CONTRACT FOR OVERNIGHT FIELD TRIPS (for ALL students)
THEODORE ROOSEVELT HIGH SCHOOL - KENT CITY SCHOOL DISTRICT PERMISSION FOR STUDENT TRAVEL (for ALL students)
EMERGENCY MEDICAL AUTHORIZATION FORM (for ALL students)
1. - All students must have an EMA form completed by a parent or guardian in Final Forms to participate in any field trip.
    1. - We will be coordinating with the clinic to get these forms and will be in touch, if we find that one is missing.
      - Please update your son or daughter's EMA form as necessary at the link following the number 3.

THE FOLLOWING FORMS MUST BE COMPLETED ONLY BY STUDENTS TO WHOM THEY APPLY

NON-PRESCRIPTION MEDICATION FORM (for students traveling with non-prescription medication)
1. - If non-prescription medication is necessary
  - One form per medication
  - Parent/Guardian signature required on (each) form
    - - ALL Medication MUST be in their original containers
      - Meds must be carried by traveling parent or group leader
      - Meds are NOT to be shared with other students under any circumstances
PRESCRIPTION MEDICATION FORM (for students traveling with prescription medication)
1. - If prescription medication is necessary
  - One form per prescription
  - Parent/Guardian signature required on (each) form
  - Doctor signature required on (each) form
    - - ALL Medication MUST be in their original containers with prescription info
      - Meds must be carried by traveling parent or group leader
      - Meds are NOT to be shared with other students under any circumstances
ALLERGY ACTION PLAN (for students requiring an action plan for allergies)
1. - Group leader checks with Clinic
    - - If allergy plan exists, nurse will provide copy
      - If no file in clinic:
        One form per allergy plan
        Parent/Guardian signature required on (each) form
        Doctor signature required on (each) form
SCHOOL ASTHMA ACTION PLAN (for students requiring an action plan for asthma)
1. - Group leader checks with Clinic
    - - If asthma plan exists, clinic will provide copy
      - If no file in clinic:
        Parent/Guardian signature required on (each) form
        Doctor signature required on form
SEIZURE ACTION PLAN (for students requiring an action plan for seizures)
1. - Group leader checks with Clinic
    - - If seizure plan exists, clinic will provide copy
      - If no file in clinic:
        Parent/Guardian signature required on (each) form
        Doctor signature required on form

PLEASE BE SURE THAT ALL EMA FORMS ARE UP TO DATE WELL BEFORE TRAVEL.
