Winter Sports Information

In order to participate in athletics at SVA, each student must have a Final Forms account that has been approved.

The completed online athletic packet including the **KHSAA sports physical must be approved before you can condition, tryout, attend open gyms or practice. 

We will not accept any hard copies of physicals or insurance card copies. Everything must be completed online.

**The KHSAA Sports Physical is NOT a 6th grade school physical. It is a separate form that the doctor must complete and then it must be uploaded to FinalForms.

Boys Basketball Tryouts

October 15th, October 16th and October 17th 

7th Grade 430-600 

6th Grade 600-730 

8th Grade 730-900

**Final Forms account must be approved by October 11th to tryout. This includes completing all required forms online plus uploading a current KHSAA Sports Physical and insurance card. 

Girls Basketball

Open Gyms: 

October 15th & 16th from 3:15-4:30 

Tryouts: 

October 21st, 22nd, and 23rd

Grades 6th-8th 3:15-5:00

Archery

Tryouts: 

October 22nd, 23rd, and 24th 

Grades 6th-8th 6:00pm-8:00pm

**Final Forms account must be approved by October 15th to tryout. This includes completing all required forms online plus uploading a current KHSAA Sports Physical and insurance card.
