The following forms and information listed below are required to register your child.

• Kentucky Immunization Certificate-Parents get form from Health Care Provider or Health Department

• Original Birth Certificate

• Medical Examination

• Copy of Child's Social Security Number

• Free Lunch Form (When appropriate)

• Proof of Residency - street address which may include utility bills to the legal guardian, rental/lease/purchase contracts, etc.

• Transcript, report card, and/or other academic records from prior school(s)

• Dental Screening Form-Due the first day of school.

• Eye Examination Form-Due the first day of school.

Brigance Testing

Within the first couple of weeks of school, your child will take the The Brigance Inventory of Early Development ii (IED-ii). While we use the word "test," it is really more of a one-on-one assessment with an adult to let us know where your child stands in 5 key areas of development in relation to his/her peers.

Those areas are:

1. Language Domain (receptive and expressive)

2. Motor Domain (gross motor and fine motor skills)

3. Academic-Cognitive (general/quantitative and pre-reading skills)

4. Daily Living Domain (self-help and prevocational)

5. Social-Emotional Domain (play skills and behavior and engagement/initiation skills)

In order to help our staff with this process and with getting to know your child, it is imperative to have these two forms back as well. There are both English and Spanish version attached.

After your child is placed in a class, you will be invited to Class Dojo, where you will communicate with your teacher and the administration. If there is anything new to know, the information will be posted in one, or all, of these locations.
