This webpage provides information for those who have been vaccinated under the framework of Keio University COVID-19 Workplace Vaccination.
A request to those who have yet to submit their vaccine vouchers (coupons).
We have closed the submission of the vaccine vouchers for the 1st and 2nd vaccinations.
We correspond directly with the local government offices regarding he vaccine vouchers for the 1st and 2nd vaccinations. For the vaccine vouchers for the 1st and 2nd vaccinations, please ask local government after September 1st.
We are still accepting the vaccine vouchers for the 3rd vaccinations.
Keio University collects vaccination vouchers (coupons) from persons vaccinated at Keio University and submits these to local government offices along with Prevaccination Screening Questionnaires.
If you did not submit your vaccination voucher (coupon) when you were vaccinated, please complete the following steps as soon as you receive the voucher from your local government office (for the 1st and 2nd vaccinations in 2021 and the 3rd in 2022.)
If you lost your vaccination voucher, please request that it be reissued at the local government offices in the municipality included on your Certificate of Residence.

• Before submitting your vaccination voucher (coupon), please make a hardcopy or take a picture of it and retain it for safekeeping.
• Please do not remove the vaccination voucher (sticker portion) from its backing paper but submit it while still attached to the backing paper.

Issuance of Vaccination Certificates (Vaccine Passports) for International Travel by Local Governments
A Vaccine Passport is a document generated through an application to your local government (municipality) office to verify your COVID-19 vaccination status for international travel. It will include your name, nationality, and the date of your vaccinations.
Beginning on July 26, 2021, local governments began accepting applications for issuance of written statements based on vaccination records for persons with valid passports for international travel.
Required documents for this application vary by municipality. Please check the respective municipality's website for details. Some local government offices may even require ID displaying an address that corresponds to that at which you were resident when you received your vaccinations (driver's license, health insurance card, etc.)
We are unable to reissue the Record of Vaccination (with stickers).

If you did not submit your vaccination voucher (coupon) for the 3rd vaccinations when you were vaccinated, the record of the 3rd vaccination is not reflected on the vaccination certificates.
Please complete the following steps as soon as you receive the voucher from your local government office (for the 1st and 2nd vaccinations in 2021 and the 3rd in 2022.)
To those who submitted their vaccination voucher (coupon) to Keio University when receiving their first vaccination but did not get their second vaccination at Keio University.
If you have submitted the first and second vaccination vouchers (coupons) to Keio University at the time of the first vaccination but did not receive your second vaccination at Keio University, we will be unable to return your vaccination vouchers to you.
If you need your second vaccination voucher, please request that it be reissued at the local government offices in the municipality included on your Certificate of Residence.
Regarding side effects
Side effects have been reported after getting the vaccine, and those include pain and swelling of the area where you got the shot, headache, joint and muscle pain, fatigue, chills, and fever. Regarding side effects, please also refer to the websites below:
