REQUIRED FORMS
PHYSICAL/MEDICAL HISTORY
-Athletes must receive a new physical (completed by a doctor) dated after May 1, 2025 to be eligible to participate during the 25-26 school year. You can print the physical form from the Rankonesport.com website or your daughter can pick on up in our office.
*Students enrolled in Athletics must have a physical completed by August 15th to remain in the athletic class period.
Once the physical and medical history pages have been completed and signed, please upload those files to your Rankonesport.com account. We will not accept any paper copies. Athletes cannot participate in any August SLJH SAC/Skills/Open gym until these forms are completed and approved by our athletic trainer (please allow 24 hours).

Log on to rankonesport.com and complete the online forms. Next school year's forms are available for you to complete NOW.
ATHLETIC FEES
Athletic Fees will need to be paid before equipment is issued. When that goes live, I will send a message out via SportsYou (late July/early August). Fee must be paid on Pay ‘n Go. No cash or checks are accepted.
PRACTICE CLOTHES
-Athletes will pick up their practice clothes after one of our skills sessions (posted in the August Schedule)in the girls locker room. Practice clothes will not be issued until the Athletic fee is paid & the required paperwork is complete.
-Extra Practice Clothes. We will the team store again the first week of school to order additional practice clothes for your daughter. These clothes will be hers to keep. Clothes that are in the school dress code can be worn to school (sweatshirts/T-shirts). Shorts are not in the dress code and can not be worn to school.

What is Athletics?
-Athletics take the place of PE in your daughter's daily schedule. In athletics, we focus on sport specific skills (in season) and endurance, speed, agility and strength (off season).My daughter just had her physical completed for cheer tryouts does she have to have another physical?
-Yes! To participate in athletics during the 23-24 school year, the physical must be dated after May 1, 2023.My daughter is new to Katy ISD how can she be placed in athletics?
-She will be evaluated for athletics during our SLJH SAC sessions in August (dates released in May). If she makes the program she will be moved into the athletic class period. This is for both incoming 7th and 8th graders.Can my daughter participate in more than one sport?
-Yes!What does it cost to be in athletics?
-KISD requires a $40 athletic fee! This covers the use of practice clothes, uniforms, bags, sweats, etc.
