RALLY - A WELLNESSWORKS PROGRAM

All current covered associates and eligible dependents (over 19 yrs. old) are invited to take the online Health Assessment at www.uhc.com.

Covered medical plan members* can each earn gift card rewards for participating in the programs below as well as coins on Rally to enter into the United Healthcare sweepstakes.

The online Health Assessment must be completed by Monday, February 28, 2022.

*All active employees currently enrolled in the Y's medical plan, pre-age 65 retirees, and spouses/domestic partners are eligible to earn incentives. Covered adult children aged 19-25 are eligible to participate in any of the programs that are offered, and will be eligible to earn coins on Rally to enter into the United Healthcare sweepstakes. Covered adult children are not eligible for the gift cards.

EMPLOYEE ASSISTANCE PROGRAM (EAP)

The Y is committed to providing associates with the information and resources they need to make informed choices about life and work issues. The EAP and WorkLife Benefit are free and confidential services provided through United Behavioral Health for associates and immediate family members

The YMCA of Greater Kansas City pays the cost for this service.
