Transcript Request
Students who need an official transcript, or need SBHS to send an official transcript to a third party (college, university, technical school, scholarship etc).
You must email judy.weisz@k12.sd.us.
The email must include
Your name (at the time of graduation),
Date of Birth
Year of Graduation
Where the transcript needs to be sent
Your signature.
You can take a picture of the request with your signature or print the form below, sign it, scan it and email it to Judy Weisz.
