All new students are requested to bring a report card or transcript of grades, and immunization records from the previous school attended. Students will not be admitted to school until and unless the school nurse has received and verified the immunization documentation.
New students must also bring a certified copy of their birth certificate or affidavit in lieu of the birth certificate as issued by the Department of Health if the original birth certificate is deemed unattainable. They will also be asked to provide their Social Security number, and race and ethnicity information. Meade School District will request the official school records from the former school.
Regular school hours will be 8:05 a.m. to 3:10 p.m.

Request for Records- This form enables us to get the information from the student's former school. This is not in lieu of the Birth Certificate and Immunizations.
Meade School District Enrollment Form- The first page of the form is strictly about the new student, the second page is referring to all people living within the household where the student lives, an emergency contact and where the student is transferring from. If there is a secondary household, please list it on the bottom of page 3 with names, phone numbers and addresses of this household along with the people who live in that household.
Immunization Authorization Form for Enrollment - This form allows access to child's immunization record in the South Dakota Immunization Information System.
Health Service Information - This form must be completed entirely to be kept on site in case of illness or emergency.
Meade School District Information Technology Acceptable Use Agreement - This form must be signed on page 2 by the student and by the guardian/parent
Free or Reduced Price Lunches- This is NOT a required form for registration but is available for all students to apply.
Parent Information - This is for your information only and subject to change yearly with the price of meals and enrollment numbers.
