WMS - New Student to DOE

In-person Registration daily from 9am - 2pm, no appointment necessary

Forms Required:

• SIS-10W - Enrollment Form

• Proof of Residency - lease agreement or utility 

• MV1 Form

• Immunization Record including TB (VAX to School flyer gives details)

• SPAV - Audio Visual Clearance

• TRUF - Technology Release

• Transcript Request (not needed if all prior school records are provided)

• Special Needs Questionnaire

• Family Household Survey

SIS-10W Forms are available translated into several languages other than English on the Hawaii DOE website.
