JNP-Family Engagement Transition Toolkit is designed especially for emerging young adults with disabilities to utilize as a resource to support the course of transition planning for postsecondary education and employment. Information has been included that will help families, caregivers, school counselors and service providers with offering guidance as you develop your academic or employment goals. It can also assist in identifying appropriate agencies, institutions and programs that may best match your needs and interests as you plan your transition.

Leadership in Disabilities & Achievement of Hawai’i: LDAH assists families with consultation on educational, work, and life opportunities for children and youth with or at risk of disabilities by empowering them and their families through screening, identification, information, training, and mentoring, and by public outreach and advocacy.

Hilopa'a - Hilopaʻa provides information and referral, technical assistance, and training for parents with children with disabilities. Hilopa'a Center is staffed by parents and friends of children with special health care needs. They provide free, confidential assistance to families and self-advocates and the professionals who serve them, and provide resources for students transitioning out of High School.
