Copy of student Birth Certificate
DOE Student's Health Record - Form 14 (hard copies can be picked up from the main office, completed by child's doctor, and returned)
Copy of Immunization Records
Copy of Tuberculin Test Results
Copy of Parent/Guardian's Photo ID
Verification of Residency
Verification of Residency Notary Form (if needed)
Copy of Most Recent Transcript/Report Card

7th Grade Health Requirements
All new and continuing students entering 7th grade must have a physical exam, also called a well-child visit, prior to attending 7th grade next school year.
Physical examinations (PE) must be completed within one (1) year before attending 7th grade.
Immunizations must meet the age, dose, and minimum intervals as described in the DOH Immunization Requirements Summary.
*NOTE: Students may be exempt from immunization requirements for medical or religious reasons.
Tuberculosis is not needed if your child has been attending a Hawaii school.
Student's Health Record (Form 14) must be completed by your student's health care provider and submitted to the main office before the first day of school. Forms can be picked up from the main office or you may print them from home from our school website.
