What's a Residency Verification Form?
Schools require proof of residence and parent or guardian identity to ensure children are enrolled in the correct school and that student records are accurate. Parents, licensed foster parents, or court-appointed legal guardians must provide a photo ID, such as a driver’s license, passport, or state ID. To verify residence, schools prefer documents like a rental or lease agreement, mortgage papers, property assessment, or a utility bill in the parent/guardian’s name. If a child lives with a relative or friend, a notarized statement along with the relative’s proof of residence may be accepted. Schools may request multiple documents, use alternative verification if needed, or conduct a home visit for questionable cases.
