PHOTO ID of Parent/Legal Guardian or Foster Parent

BIRTHDATE & NAME VERIFICATION of student - Birth Certificate or Passport

 PROOF OF RESIDENCE: mortgage document, rent/lease agreement or acceptance letter, electricity/water/gas utility bill, or notarized letter

 ENROLLMENT FORMS:

• SIS-10W

• MVA Questionnaire (MV-1)

• Home Language Survey (HLS)

• Supplemental Kindergarten Enrollment Form (SKEF)

• Kinder Camp (for new Kindergarten student)

 HEALTH RECORDS:

• Physical Examination

• Immunizations

• TB Clearance - required before attendance

 LEGAL DOCUMENTS: Power of Attorney, Guardianship, Child Custody, Caregiver Consent, Legal Name Change
