Step 2: Fill out the UPLINK Registration Form (Upload in the online google form if you are completing the google form) or email to g-296-em-uplink@k12.hi.us

If you are not able to complete the online version, please print the two following documents. Completed forms may be emailed to g-296-em-uplink@k12.hi.us or dropped off to our Ewa Makai Front Office
