If you are not able to complete the online version, please print the two following documents. Completed forms may be emailed to g-296-em-uplink@k12.hi.us or dropped off to our Ewa Makai Front Office
Paper version of the UPLINK Application and the UPLINK Registration is available at our Ewa Makai Front Office during business hours. You may turn in the completed UPLINK Application and UPLINK Registration Forms to our purple UPLINK box next to the copies of the forms.
We will accept the UPLINK Application and Registration Form by the following:
2526 UPLINK Application (google form) with the UPLINK Registration Form uploaded into the application OR
2526 UPLINK Application (google form) and emailing the UPLINK Registration Form to g-296-em-uplink@k12.hi.us OR
2526 UPLINK Application pdf/paper version and UPLINK Registration form email to g-296-em-uplink@k12.hi.us OR
2526 UPLINK Application paper version and UPLINK Registration form paper version dropped off to the UPLINK box at our Front Office
If you fail to turn in an UPLINK Application Form AND UPLINK Registration Form this may impact your UPLINK Registration. If you have any questions you may email g-296-em-uplink@k2.hi.us. You will receive a response for the UPLINK team within 72 business hours.
