New Student Registration 

You will need to have the following information to complete the application:

• 1) Proof of residency – 2 forms which must include 1 of each:

  • Lease agreement or mortgage statement 

  • Current utility bill, bank statement, pay stub, etc. showing address in SRMS boundaries. If you are living with family, please bring a notarized Affidavit of Rental or an Affidavit of Occupancy.

• 2) You must also have available:

  • Withdrawal paperwork from previous school.

  • Birth Certificate

  • Immunizations
