Registration Dates:

Any child who will turn four on or before September 1, 2026 is eligible to enter our half-day 4K program in the fall of 2026.

Any child who will turn five on or before September 1, 2026, is eligible to enter 5K/Kindergarten in the fall of 2026.

If your child has never attended in the Johnson Creek School District AND families don't current have a child enrolled at Johnson Creek, you will need to complete the form below:

REGISTRATION FOR FAMILIES WITH CURRENT JOHNSON CREEK DISTRICT STUDENTS

If your family has students already enrolled in the Johnson Creek School District at any grade level, please login into your Skyward Family Access. Click the button on the top left side of the home page that says "New to JCSD Registration". Clicking this button will take you to the registration page to fill out for your new student!

Documents Needed to Register

Proof of Residency - Current utility bill (gas, electric, cable or city/village water with name and current address), signed lease/rental agreement, property tax bill, current wage statement, government correspondences (W2 cash benefits, food share, housing assistance, SSI).

Birth Certificate - certified original

Immunizations - An Immunization Record is required for all students. Immunization Records can be submitted by a parent/guardian or Physician's Office. Email or fax to 920-541-4851.
*6th grade students need their TD/TDAP Booster
