Dear Student Athletes & Parents,
This communication regarding the Missouri State High School Activities Association’s (MSHSAA) eligibility requirements, as well as Jennings Senior High School’s “Warrior Way” criteria. In order to be eligible to participate in high school athletics in the State of Missouri, students must:
You must be enrolled in courses offering 3.5 units of credit· You must have earned 3.0 units of credit the preceding semester (therefor 2 or more semester Fs means a student is ineligible during the next semester)

JSH Warrior Way Criteria:
Maintain passing grades in all classes, no Ds or Fs.
Maintain three or less disciplinary infractions in a semester; no severe discipline (10 days, etc.).
Maintain 85% or higher attendance.
Must be present at school on practice and game days in order to participate. That includes skipping class.
We will attach your student’s individual grade report and transcript for your review. Please speak with your student and encourage them to come to school every day and on time to improve their attendance prior to the conclusion of the first semester in order to participate in the above mentioned activities.

PHYSICAL INFORMATION:
Junior & Senior High Parents/Guardians:
Students must have a current physical on file in order to be physically eligible to participate in athletics.
Students who need an updated physical can make an appointment with the SPOT Center located inside of Jennings Senior High School. Parents can make an appointment with the SPOT by calling 314-653-8141. Student's must have their parents sign a SPOT CONSENT FORM in order to be seen. The link for JSPOT Consent Forms and Physical Portion is listed below. The JSPOT Medical Providers are here on 1st and 3rd Tuesdays, and every Thursday. Once parents have completed both consent forms, they will be scheduled for an appointment.

FINAL FORMS INFORMATION:
Jennings Athletic Department is promoting a link called "Final Forms" to streamline the athletic process for K-12 by eliminating the need for paper forms. Established by the Missouri State High School Athletic Association (MSHSAA), this forms portal (JSD Participation Information - Final Forms) makes it easier for parents/guardians to upload students' physicals and other required information. All mandatory information will be collected online.
Click on the link and set up an account in Final Forms. The physical examination must be turned in before participating in any sport. If you created a Final Forms Account last year, you are all done! Physicals are good for two years.
Obtain a physical from your physician or from another community source, such as the Fairview Clinic in Jennings (designated dates will be posted), which is FREE to District students during designated hours. Other places, such as The Spot, Walgreens, CVS or Urgent Care, may charge. We will post a list of free sites on the Athletic web page.
Approval from the Athletic Department will come once all forms are in and have been reviewed.
