Individual                Learning Plan

What is the ILP?

Our ILP is integrated into our career academies, our classrooms, and our advisory classes. Students are assigned to an advisory group that meets every other week during SAT to develop and implement their ILP using a Transition Readiness Scorecard and a SAT notebook. The ILP and advisory process reflects the inclusion of career planning content and life and career options to ensure student success beyond high school. The ILP and advisory process ensures exploration/preparation in career paths and post-secondary education.Kentucky state law requires students to review and revise their Individual Learning Plan (ILP) annually. It will then be reviewed by a staff member.

How Can Parents Access Their Student’s ILP?

To access the ILP, students should log in to JCPS Digital Backpack of Skills. Students should have a copy of their Transition Readiness Scorecard in their Prepared and Resilient Learner folder. Students have been updating this as part of their regular classes. Please note, the ILP is an evolving document that will follow students through high school. There may be sections of the Transition Readiness Scorecard that a student is not yet ready to complete or check off but will be completed in other years of their high school career.

If you want to see blank copies of the Transition Readiness Scorecard and SAT Notebook, click the links below. Students can also make copies of these files to add to their Digital Backpack if they do not already have them.
