A sixth grade entry physical is not a sports physical
The link below contains the KHSAA and JCPS athletic forms. All forms must be FULLY completed before the first day of tryouts or practices. In elementary school, your child only had to complete and submit the sports physical forms completed by your health care professional, but in MS/HS we have a few more forms. Once again, you will find all forms to participate in middle school sports below.
