To sign up for an athletic team, you must create your Final Forms account.  

FinalForms is an online data management platform designed to streamline athletic registration. Through this system, parents can complete student athletic sign-ups, securely upload mandatory physical forms, and receive direct updates and communications from the athletic department. 

FINAL FORMS Upload ONLY the "MEDICAL ELIGIBILITY FORM GE04" (page with student's name, date of service, and medical clearance with a doctor's signature) and the Parent Permission Release Form- MUST be notarized. 

Steps to Create Your Final Forms Account

1. Click Final Forms above 

2. Next page, click Login (top right corner)

3. Find Your School District-- Type Jefferson County Schools

4.  If you are new to Final Forms, create a new account

5. Follow directions throughout

Make sure to select the sport(s) your child is interested in participating in. All communication regarding tryout dates, times, and locations will be shared via email through Final Forms.
