Print out these forms, complete them, have your physician complete them, and turn them into your coach BEFORE play!
Every student must have a complete set of forms in order to participate. Please click on the link above for all the pages required, including the physical forms for your doctor.
*One form must be notarized! Don't have a notary? Don't worry...our school secretary, Beth Schultz, is a notary. She can help you!
Turn in all pages to your coach or Mrs. Brown.
You can also scan and email the completed physical to Becky Brown @ becky.brown@jefferson.kyschools.us or you can fax them to (502) 313-3483
