• Physical /Wellness - SCHC (Also Needed)  - Physicals are provided, but completion of paperwork is necessary. Two forms are linked. To get a free physical at school, both must be filled out and brought to school before the physical appointment. If you go to your own doctor, only KHSAA form is needed.
