2025 Sports Physical & Registration Information

• 1. Registration is now open! Please use our new online sports registration platform.

  2. Here’s what you need to know to get your student-athlete ready:

     1. All athletes must register online and have a current CHSAA physical on file before the first official day of tryouts or practice — no exceptions.

     2. Physicals are valid for 365 days, so please confirm your student’s physical will remain current through the full season.

     3. The CHSAA Medical Eligibility Form is the only accepted physical form — all others will be declined.
