Transition
Planning for Adulthood
In Special Education, the transition from school to life after high school is an essential process, focusing on creating a plan for a fulfilling adult life. This plan is developed by the IEP team, which includes the student, teachers, support staff, vocational staff, and families. The primary areas of focus during this transition are employment, education or further training, and independent living. Though it might feel emotional for some, it is also a time to celebrate new opportunities and growth.

Transition Planning
Transition planning is crucial because it prepares students for life after high school by starting the planning process on or before their 14th birthday. This planning:
Gathers important information from the student to prepare for the future.
Focuses on graduation and future goals.
Aligns with the student's interests.
Contributes to the selection of electives in their four-year academic plan.
