How to complete a request:

1. Determine item availability by using calendar below

2. Complete and submit "Teacher Agreement Form" located in top menu

3. Complete Request Form below (Maximum 2 weeks check out)

4. An IU5 employee will contact you to discuss/arrange necessary training and to schedule pick up and return of items
