Referral Pathway:

At Integrity Youth Homes, we carefully review each referral to ensure the youth’s needs align with our program and level of care.
We provide a safe, structured, and trauma-informed home environment for male and female youth, ages 17 and under.

Before Submitting a Referral

To help determine if our home is the right fit, please note:

• We are a Level 1 Behavioral Health Residential Facility (BHRF) — serving youth who benefit from a supportive, therapeutic environment but do not require frequent physical intervention or intensive stabilization.

• We do not perform holds or restraints.

• We cannot accept youth with recent self-harm (e.g., cutting) or behaviors requiring physical holds.

• A history of sexualized behavior, aggression, or legal involvement must be disclosed to ensure the safety and compatibility of all residents.

• Past self-harm or behavioral incidents are not automatic disqualifiers if sufficient time has passed and the youth has demonstrated stability.

If you’re unsure whether a youth may qualify, please reach out — we’re happy to discuss the clinical history before records are sent.

Step 2: Submit Clinical Packet for Review

Please fax to (888) 920-2173 or email directly.

Helpful documents for review include:

1. Face sheet (include insurance information)

2. Most recent psychiatric evaluation

3. Recent incident reports

4. Psychological testing (if applicable)

5. Current medication list (MAR)

6. Recommended level of care

7. Recent progress notes (minimum 2 weeks)

8. Hospital discharge summary (if applicable)

9. Behavioral and legal history summary (including aggression, sexualized behavior, or law-enforcement involvement, if any)

10. Immunization  records needed for medical history and school enrollment.
    
    

Step 3: Clinical Review & Next Steps

Our clinical team reviews all documentation to ensure the youth’s needs align with our Level 1 BHRF model and current peer mix.
We’ll contact you promptly to discuss suitability and, if appropriate, arrange admission details.
